Two-dimensional and three-dimensional bone defects require special surgical reconstruction procedures to ensure an acceptable functional and aesthetic result in the long term. Long-term success depends on an accurate pre-operative diagnosis, atraumatic surgery, amount of vascularization and vitality of the grafted area, correct implant geometry and position, as well as systematic soft tissue augmentation, also improving the quality of the surrounding gingiva.

Autogenous bone grafts harvested from the jaw and used after the split bone block (BOD) technique offer many possibilities for intraoperative installations, leading to high vascular support of the grafted area, which is important for stable long-term results. Dividing the thick cortical block into 2 or 3 thin blocks increases the number of blocks, allowing the reconstruction of a larger atrophic ridge and providing better adaptation to the recipient site with individual determination of the width and volume of the grafted area.

Filling the spaces and gaps between the thin block and the remaining ridge with particulate bone chips reduces the time needed for graft revascularization, improving its vitality in relation to the original thick block. Implants inserted into this grafted bone showed, in the long term, an osseointegration similar to that of implants placed in non-grafted bone. Different pedicles or free soft tissue grafts guarantee the necessary soft tissue volume
